Hillcrest Health Systems, the area's premier provider of services for aging adults, is currently seeking a qualified Clinical Admissions Coordinator candidate to join our care navigation team. Our vision is to be a national leader, providing integrated services to aging adults while enhancing the lives of those we serve every day. We are the area's largest continuum provider of healthcare services to aging adults. Our continuum of services includes IL, AL, Memory Support, LTC, Post-Acute Care, outpatient Physical Therapy, Home Care and Hospices services to name a few.

We are seeking an outgoing, energetic sales minded Clinical Admissions Coordinator (formally titled "Care Navigator") to support our census growth and admissions efforts .

Responsibilities for Clinical Admissions Coordnator:

  • Increasing census through outreach efforts at local hospitals and physician clinics
  • Building trustworthy relationships with physician office teams, social workers, and discharge planners
  • Meeting prospective patients and their families and explaining the process of admission to all of Hillcrest Health Services.

Requirements for Clinical Admissions Coordinator:

  • 2 year's experience in an acute, post-acute,or LTC setting working with admissions or healthcare sales
  • Must be organized, friendly, goal-oriented, self-motivated, willing to travel to various hospitals during the workday
  • Able to multi-task
  • Acute or Post-acute care experience as a nurse preferred, but not required
